1. Applicants can go
directly to our office to fill in the application
form and pay the annual membership fee, HK$50, (Recipients
of Comprehensive Social Security Assistance will
be exempted from membership fee. Please attach a
copy of the notification from the Social Welfare
Department when returning the application form)/or
2. Applicants can phone our association for a form,
and send the completed form to us with a HK$50 cheque
made payable to "Hong Kong Stoma Association."
